STEP 1: PRE TRAVEL ASSESSMENT You and your travel Partner will complete this step together (allow a maximum of 3 - 5 minutes to complete). This step must be completed in one session, as you will not be able to save and resume later. Remember to click on the SUBMIT button once completed. No document uploads required in this step. STEP 2: APPLY FOR YOUR PASSPORT Follow this step only if you do not have a valid passport. Click here Kindly apply for your passport and make your booking before : 23rd August 2024 This will ensure sufficient time should there be any delays in the issuing passports. |
STEP 3: REGISTRATION You and your travel Partner will complete this step together (allow a maximum of 10 - 15 minutes to complete). This step must be completed in one session, as you will not be able to save and resume later. Remember to click on the SUBMIT button once completed. Documents required to complete registration for Winner, Host & Accompanying Person: 1. Passport and a copy of your passport for Upload. (if you do not have your passport, you can complete the registration and email a copy on receipt. 2. Yellow Fever Certificate for Upload, only if you have a valid card. 3. Head & Shoulders photograph for Upload, does not have to be a professional photo. 4. Copy of ID and Utility Bill no older than 3 months Upload (Winners and Hosts only). STEP 4: YELLOW FEVER VACCINATION ADMINISTRATION FOR WINNERS / HOSTS / ACCOMPANYING PERSONS CAPE TOWN Yellow Fever Vaccinations will be available for administration at Mkem Pharmacy between the 30th September and the 11th October 2024. The Travel Clinic will have a list of all your names, you simply present yourself, communicate that you are part of the Momentum Corporate Group and sign next to your name. MKEM Pharmacy Address: 224 Durban Road, Bellville (1st Floor) Travel Clinic Operating Hours Monday – Friday: 8am – 6pm - No appointments, it is walk in. Saturday: 8am to 1pm - By appointment only. Closed on Public holidays ***You will need your ID or Passport*** GAUTENG The Travelling Clinic will Visit your Offices as per below schedule. You will receive a mail to book your date. Centurion Office, Thursday, 3 October 2024 Venue : Meeting Room GF-10 Sandton Office, Wednesday 9 October 2024 Venue : Sandton 129R 1F1 Pula Vaccines will be administered on a first come basis.
